Public comment period for HCBS Settings Rule Revised Statewide Transition Plan

Purpose: DHS requests comments on proposed changes to the HCBS settings rule statewide transition plan

Comments sought on revised HCBS transition plan

DHS plans to submit a revised statewide transition plan (STP) to the Centers for Medicare and Medicaid Services (CMS) after a 30-day public comment period. The statewide transition plan describes how Minnesota will meet the new federal rule for home and community-based services (HCBS) settings.

Based on early CMS guidance, we developed and submitted an initial HCBS settings rule statewide transition plan in January 2015. CMS responded with new guidance and expectations over the course of 2016.

Based on this additional CMS guidance, we made significant amendments to the STP to address gaps in the initial plan, specifically in the areas of site-specific assessment, validation and remediation.

Please review the following documents and submit comments to HCBS.Settings@state.mn.us no later than 4 p.m. Nov. 3, 2016:

Systemic assessment crosswalk for elderly waiver (EW) and alternative care (AC) (PDF)

Submit comments to HCBS.Settings@state.mn.us. Comments must be submitted no later than 4 p.m. Central Time on Nov. 3, 2016. We will also accept comments by U.S. Mail. Mail written comments to:
